Adoption Application

Please fill out and submit this form for consideration.
If you prefer, you can use a printable copy of this form that can be mailed to us.
Form file size is 40K -- any problems, email: website@hotwaterrescue.com

PLEASE NOTE: Due to the volume of applications, we are only able to contact those applicants who seem best-suited for each dog.

If you've never worked with/adopted from a rescue group before, you may start to get anxious and wonder why it seems to take so long to hear back from us. Please know that we appreciate your patience while you wait for your application to be processed. While it may appear to be taking a long time - a representative of Hot Water Rescue will contact you as soon as they are able. Our organization is 100% staffed by volunteers who donate their time to helping these dogs while juggling full-time jobs and family responsibilities. We assure you that we are all working as hard as we can to process applications, and contact our applicants, as quickly as possible.

Be sure to answer each required field (required fields are marked with an *) leaving any required field blank will result in an error message and you will have to start the application all over again. Thank you!

We would like to know their age, gender, breed, size, general temperament towards other animals.

What type of home do you live in: house, apartment, condominium, etc. Please describe your home to us. (Do you have a yard; if so, is it fenced?) Does your family live in an urban, suburban, or rural setting? Most importantly, do you feel as though your situation is the best situation possible for the dog that you are applying for?

* 17. Are you financially able to follow our feeding requirements for the dog adopted?

(We firmly believe that nutrition is what can ensure that a dog will live a long, healthy life. Accordingly, we want our dogs to be placed in homes which will prioritize their dog's health. You can read more here: http://www.hotwaterrescue.com/nutrition.htm).

We often get many applications for the same dog -- in the event that the dog you wish to adopt has already been adopted out, would you like us to keep your application on file for when another similar dog becomes available? If so, please let us know the characteristics you are looking for.
